2021, Annals of Diagnostic PathologyCitation Excerpt :The explant lung pathology of Case 4 showed features of a UIP pattern of chronic IPF and granulomatous lesions consisting of cavities filled with fungal hyphae of Aspergillus sp. morphology and foci of invasive fungal hyphae into adjacent lung tissue. These findings are typical of chronic necrotizing pulmonary aspergillosis characterized by necrotizing granulomas with extensive parenchymal consolidation and bronchiectatic cavities [22,23]. A pre-transplant clinical diagnosis of pulmonary fungal or non-tuberculous mycobacterial infection was suspected in 3/4 (75%) of patients.
