Specific causes include Williams–Campbell syndrome (bronchial cartilage deficiency),7 11 12 tracheobronchomegaly (Mounier–Kuhn syndrome),1 6 Marfan's syndrome,10 late presenting H-type tracheobronchial and oesophagobronchial fistula.3 Bronchiectasis has been reported in congenital lung malformations such as sequestration2 8 or rarely with a rib malformation.9
