Abstract
Four hundred and fifteen patients with pulmonary tuberculosis were admitted to a controlled study of a year's treatment, on an out-patient basis, with one of the following two fully supervised regimens:

SH/SHOW. Streptomycin 1 g or 0·75 g plus isoniazid 400 mg administered daily for the first four weeks, followed by streptomycin in the same dosage plus isoniazid 13 mg/kg or 17 mg/kg body-weight, administered once a week for the rest of the year. Pyridoxine 6 mg was incorporated in every dose of isoniazid.

SPH/SPHOW. Streptomycin, isoniazid and pyridoxine in the same dosages as in the SH/SHOW regimen, plus sodium PAS 6 g throughout the year, all the drugs being administered daily for the first four weeks and once a week for the rest of the year.
The regimen, the streptomycin dosage and the once-weekly isoniazid dosage were allocated at random for each patient.
The main analyses in this report concern 359 newly-diagnosed patients (181 SH/SHOW, 178 SPH/SHHOW) with cultures sensitive to isoniazid and streptomycin on admission. About 90 per cent of the patients had cavitated disease and a positive sputum smear on admission, and 40 per cent were rapid inactivators of isoniazid. The condition on admission was similar for the two series.
Two patients (both SH/SHOW) died of tuberculosis and four (all SH/SHOW) had their chemotherapy changed on account of radiographic or clinical deterioration in the presence of a positive sputum. At one year, 85 per cent of the SH/SHOW and 87 per cent of the SPH/SHHOW patients were classified as having a favourable response, mainly on the basis of culture results at 10, 11 and 12 months. Among those who had an unfavourable response, approximately half had responded well initially but had a bacteriological relapse by one year. Considerable or exceptional radiographic improvement was shown by about three-fourths of the patients in each series, and cavitation had disappeared in about half.
Resistance to isoniazid was observed at one year in 8 per cent of the SH/SHOW and 5 per cent of the SPH/SPHOW patients, and resistance to streptomycin in 8 per cent and 2 per cent, respectively. It is concluded that the addition of PAS to the SH/SHOW regimen did not confer an appreciable benefit.
Slow inactivators of isoniazid responded considerably better than rapid inactivators, the proportions with a favourable response at 1 year being 93 per cent and 72 per cent respectively in the SH/SHOW and 95 per cent and 76 per cent respectively in the SPH/SPHOW series. The relative insufficiency of isoniazid in the rapid inactivators was manifested by less efficient elimination of isoniazid-sensitive organisms and greater frequency of streptomycin resistance. The duration of coverage with a bacteriostatic concentration of isoniazid (0·2 μg/ml) following a once-weekly dose and the total exposure to isoniazid (expressed as the area under the time-concentration curve) were substantially higher in the slow inactivators (30–34 hours; 85–111 units) than in the rapid inactivators (14–15 hours; 36–51 units). The peak concentration in the former, however, was only 22 per cent higher than that in the latter, the difference being of the same order as that between SPH/SPHOW and SH/SHOW patients, and isoniazid high-dosage (17 mg/kg) and low-dosage (13 mg/kg) patients. These findings suggest that response to once-weekly chemotherapy with isoniazid is largely determined by the duration of coverage and the total exposure to isoniazid.
Streptomycin 0·75 g (approximately 20 mg/kg body-weight) was therapeutically as effective as 1 g and less toxic. The isoniazid dosage of 17 mg/kg in the once-weekly phase appeared to be slightly more effective than the dosage of 13 mg/kg, but only in rapid inactivators.

Les principales analyses de ce rapport portent sur 359 tuberculeux nouvellement diagnostiqués (181 SH/SHOW, 178 SPH/SPHOW) ayant a l'admission des souches sensibles a l'isoniazide et a la streptomycine. 90 % de ces malades avaient une tuberculose excavée et une expectoration positive a l'examen direct lors de leur admission dans l'enquête et 40 % étaient inactivateurs rapides de l'isoniazide. L'état des malades a 1'admission était semblable pour les deux series.
Deux malades (tous deux SH/SHOW) moururent de tuberculose et pour quatre malades (tous SH/SHOW) on changea la chimiotherapie après constatation d'une détérioration radiologique ou clinique avec presence d'une expectoration positive. Après 1 an, 85 % des malades du groupe SH/SHOW et 87 % du groupe SPH/SPHOW ont été considérés comme répondant favorablement au traitement en se basant principalement sur le résultat des cultures a 10, 11 et 12 mois.
Parmi les malades ayant repondu défavorablement, la moitie a peu pres avaient bien répondu au depart, mais eurent une rechute bactériologique avant la fin de l'année de traitement. Une amelioration radiologique considerable ou exceptionnelle est apparue chez environ les trois quarts des malades de chaque série tandis que l'excavation avait disparu chez a peu pres la moitie des sujets.
Une resistance a l'isomazide a été observée au bout d'un an pour 8 % des malades SH/SHOW et 5 % des malades SPH/SPHOW, et une resistance a la streptomycine chez respectivement 8 % et 2 % des malades. On peut done conclure que l'addition de PAS au régime SH/SHOW ne confère pas un benefice appreciable.
Les inactivateurs lents de l'isoniazide ont repondu bien mieux au traitement que les inactivateurs rapides, la proportion de réponses favorables au bout d'un an étant respectivement de 93 % et 72 % dans le groupe SH/SHOW, et 95 % et 76 % respectivement dans le groupe SPH/SPHOW. L'insuffisance relative d'isoniazide chez les inactivateurs rapides s'est traduite par une moins bonne disparition des bacilles sensibles a l'isoniazide et par une fréquence plus grande de la resistance a la streptomycine. La duree de la couverture par une concentration bactériostatique d'isoniazide (0,2μg/ml) après la prise hebdomadaire d'isoniazide et l'exposition totale a fisoniazide (exprimée comme la surface comprise sous la courbe des concentrations en fonction du temps) étaient nettement plus importants chez les inactivateurs lents (30–34 heures; 85–111 unites) que chez les inactivateurs rapides (14–15 heures; 36–51 unites). Le pie des concentrations chez les premiers était cependant seulement de 22 % plus haut que chez les seconds, la difference étant du même ordre que celle existant entre les malades du groupe SH/SHOW et ceux du groupe SPH/SPHOW, ainsi qu'entre les deux groupes de malades a dosage d'isoniazide élevé (17 mg/kg) et bas (13 mg/kg). Ces résultats suggèrent que la réponse a un traitement d'une fois par semaine par isoniazide est grandement déterminée par la duree de couverture et 1'exposition totale a 1'isoniazide.
La dose de 0,75 g de streptomycine (environ 20 mg/kg de poids corporel) s'est revelee aussi efficace et moins toxique que celle de 1 g. La dose d'isoniazide de 17 mg/kg de poids corporel dans la phase d'administration d'une fois par semaine s'est revelee un peu plus efficace que celle de 13 mg/kg, mais seulement chez les inactivateurs rapides.
Resumen
Cuatrocientos quince pacientes con tuberculosis pulmonar se incorporaron a un estudio con un ano de tratamiento, en forma ambulatoria, con los siguientes esquemas supervisados: SH/SHOW — Estreptomicina 1g. 6 0,75 g con 400 mg de isoniazida, diariamente las primeras 4 semanas, seguido de estreptomicina a igual dosis con isoniazida 13 mg/Kg ó 17 mg/Kg, una vez por semana hasta completar un aho. Se agregó 6 mg de piridoxina a la isoniazida.
SPH/SPHOW — Estreptomicina, isoniazida y piridoxina en las mismas dosis que en el esquema SH/SHOW, con 6g de PAS todo el ano; todas las drogas se dieron diariamente las primeras 4 semanas y luego una vez por semana hasta completar el afio.
El tipo de esquema, la dosis de estreptomicina y la de isoniazida fueron establecidos alazar.
Se consideran en esta comunicación 359 pacientes nuevos (181 SH, 178SPH) con bacilos sensibles inicialmente a estreptomicina e isoniazida. Alrededor del 90 % de los pacientes eran cavitarios y con baciloscopia positiva initial y el 40 % eran inactivadores rápidos de la isoniazida. Los otros elementos clinicos eran análogos en las dos series.
Dos pacientes (SH) murieron de tuberculosis y 4 (SH) debieron cambiar su tratamiento debido a desmejoria radiologica o clinica con baciloscopia positiva. Al cabo de un afio, el 85% del grupo SH y el 87% del grupo SPH tuvieron una evolución favorable, con cultivos negativos a los meses 10, 11 y 12. Entre los que evolucionaron desfavorablemente, la mitad habian respondido bien al comienzo pero presentaron luego una recaida bacteriologica. Hubo mejoria radiográfica marcada en las tres cuartas partes de los pacientes de cada serie y las cavidades se cerraron en la mitad de los casos. Se observó resistencia a la isoniazida al cabo de un afio en el 8 % del grupo SH y en el 5 % del grupo SPH, y resistencia a la estreptomicina en el 8 % y 2 % respectivamente. Se concluye que el agregado de PASS al esquema SH no aportó un beneficio apreciable.
Los inactivadores lentos de la isoniazida respondieron mucho mejor que los inactivadores rapidos, y es asi como al cabo de un afio la proportion de respuestas favorables fué de 93 % y 72 % en el grupo SH, y de 95 % y 76 % en la serie SPH. La insuficiencia relativa de isoniazida en los inactivadores rapidos se demostró por una menor elimination de gérmenes isoniazida-sensibles y mayor frecuencia de resistencia a la estreptomicina. Cuando se daba una dosis por semana, la duration de la protection con concentraciónes bacteriostáticas de isoniazida (0·2 μm/ml) y la exposition total a la isoniazida (expresada por el area de la curva tiempo-concentration) eran mucho mas elevadas en los inactivadores lentos (30–34 horas; 85–111 unidades) que en los inactivadores rapidos (14–15 horas; 36–51 unidades). La concentration pico en los primeros fu6 solamente 22 más alta que en los ú1timos, siendo la diferencia del mismo orden que la de los pacientes de las series SPH y SH y la de los que recibian 17 mg/Kg y 13 mg/Kg de isoniazida. Estos hechos sugieren que la respuesta a una dosis semanal de isoniazida está principalmente determinada por la duration de la protection y por la exposition total a la isoniazida.
La estreptomicina en dosis de 0·75g (20 mg/Kg aproximadamente) fu6 tan efectiva como 1g, y menos tóxica.
La dosis de 17 mg/Kg de isoniazida, una vez por semana, pareció sermás efectiva que la dosis de 13 mg/Kg, pero solamente en los inactivadores rapidos.
Zusammenfassung
Vierhundertundfünfzehn Patienten mit Lungentuberkulose wurden einer kontrollierten Studie zugewiesen. Es wurde ein Jahr lang ambulant mit einem der beiden geschilderten Regime voll iiberwacht behandelt:

Das Regime, die Höhe der Streptomycindosis and der Isoniaziddosis bei einmaliger Gabe pro Woche wurde nnach Zufallsregeln bestimmt. Die abschließende Analyse in diesem Bericht betrifft 359 Patienten mit frisch diagnostizierter Tuberkulose (181 SH/SHOW, 178 SPH/SPHOW) and kulturell nachgewiesen, im Beginn gegen Isoniazid and Streptomycin voll sensibel. Bei etwa 90% lag bei der Aufnahme eine Kaverne vor und ein positiver Sputumausstrich; 40% waren schnelle Inaktivierer von Isoniazid. Sonst waren die Bedingungen im Beginn weitgehend identisch.
Zwei Patienten (beide SH/SHOW) starben an Tuberkulose. Vier weitere (ebenfalls SH/SHOW) benötigten eine Therapieänderung, da es zu einer röntgenologischen oder klinischen Verschlechterung mit positivem Sputum gekommen war. Nach einem Jahr wurde der Verlauf bei 85 % der SH/SHOW - and 87 % der SPH/SPHOW — Patienten als günstig beurteilt, im wesentlichen auf Grund der Kulturergebnisse nach 10, 11 and 12 Monaten. Unter den Fallen mit ungünstigem Verlauf hatte etwa die Halfte im Beginn gut angesprochen, aber nach einem Jahr trat ein bakteriologischer Ruckfall auf. Betrachtliche oder auβergewöhnliche Verbesserungen im Rbntgenbild fanden sich in beiden Serien bei etwa drei Vierteln. Zum Kavernenschluß war es in etwa der Halfte gekommen.
Resistenz gegen Isoniazid fand sich nach einem Jahr bei 8 % der SH/SHOW - and bei 5% der SPH/SPHOW — Patienten, and Resistenz gegen Streptomycin in 8 % bzw. 2 %. Die zusätzliche Gabe von PAS zum SH/SHOW-Regime ließ keinen nennenswerten Nutzen erkennen.
Langsame Inaktivierer von Isoniazid sprachen auf die Behandlung erheblich besser an als schnelle Inaktivierer. Ein günstiges Ergebnis nach einem Jahr gab es in der SH/SHOW Gruppe in 93 % and 72 %, in der SPH/SPHOW - Gruppe in 95 % and 76 %. Die geringe Wirkung von Isoniazid bei schnellen Inaktivierern manifestiert sich in der geringeren Beseitigung isoniazidempfänglicher Erreger and der größeren Häufigkeit der Streptomycinresistenz. Nach diesen Befunden hängt das Ansprechen auf eine Chemotherapie miteiner Gabe von Isoniazid pro Woche von der Dauer der Deckung und der gesamten Exposition gegen Isoniazid.
0·75 g Streptomycin (etwa 20 mg/kg Körpergewicht) war therapeutisch ebenso wirksam wie l,0 g, aber weniger toxisch. Die Isoniaziddosis von 17 mg/kg in der Phase der einmaligen Behandlung pro Woche erwies sich nur bei schnellen Inaktivierern als wirksamer denn eine Dosis von 13 mg/kg.

2014, Handbook of Pharmacogenomics and Stratified MedicineTuberculosis (TB), which is caused by Mycobacterium tuberculosis, is one of the major public health problems worldwide. Cure rates of >95% can be achieved with modern anti-TB treatment, which comprises short-course chemotherapy regimens containing isoniazid (INH), rifampicin (RMP), pyrazinamide (PZA), and ethambutol (EMB) for patients with drug-susceptible TB strains. Despite the availability of potent drugs, some patients do not respond to treatment or develop serious adverse effects.
Although many nongenetic factors influence the effects of medications, interindividual differences in drug response have been linked to sequence variants in genes encoding drug-metabolizing enzymes, drug transporters, or drug targets. Pharmacogenomic studies of drugs used in the treatment of TB can promote understanding of intersubject variations in treatment response. In this chapter, we look at pharmacogenomic data on anti-TB drugs from different settings to provide better insight into the role of pharmacogenomics in the treatment of TB, thereby enhancing efficacy and limiting toxicity of existing anti-TB medications.
A review of different studies has established the importance of pharmacogenomics in TB. Studies from different settings show that pharmacogenomics plays a significant role in INH metabolism, affecting both treatment efficacy and ADR frequency. There are limited data on single-nucleotide polymorphisms that influence plasma RMP concentrations. No data are available on the other first-line drugs, EMB and PZA. It would be useful for treating physicians to incorporate pharmacogenomics in clinical TB trials to understand the factors that influence therapeutic success and the occurrence of adverse drug reactions.

Attention is drawn to the value of the coloured urine as a reminder to patients whether or not they have taken their due dose of rifampicin-containing medicine.

The potential clinical significance of the isoniazid acetylator phenotype in the treatment of pulmonary tuberculosis
1984, TubercleIn the rural areas of Hong Kong outpatient treatment is organised from general primary health-care clinics which are visited only once a week by chest clinic staff. A daily 18-month, self-administered regimen of isoniazid and sodium PAS supplemented by daily streptomycin injections for the first 3 months has been the standard regimen, the patients attending a clinic once a month to collect their oral drugs, even during the first 3 months.
In 1979 a new rural service programme of fully supervised chemotherapy was introduced for a trial period of 1 year. Patients were to be admitted to hospital initially for up to 2 months for daily treatment with streptomycin, isoniazid, rifampicin, pyrazinamide, and ethambutol, followed by once-weekly streptomycin, isoniazid, rifampicin, and ethambutol upto 12 months, fully supervised in the primary health-care clinics.
The therapeutic results achieved with this new regimen were excellent. Among 101 patients with one or more positive sputum smears or cultures and 24 with all their smears and cultures negative pretreatment, who were assessable up to 30 months, there were no bacteriological failures during chemotherapy and only one bacteriological relapse after stopping. However, the regimen had a major operational defect in that 41 % of the 263 patients treated during the year had to be prescribed standard chemotherapy, which was not fully supervised, instead of the study regimen, 37 % because they refused an initial period of hospital admission. The therapeutic results were far less satisfactory with the standard regimen.
Dans les zones rurales de Hong Kong le traitement ambulatoire est organisé à partir des centres polyvalents de soins de santé primaires lesquels ne sont visités qu'une seule fois par semaine par le personnel du dispensaire de pneumologie. Un régime quotidien de 18 mois, auto administré, comportant isoniazid et PAS sodium, renforcés durant les 3 premiers mois par une injection quotidienne de streptomycine a constitué le régime standard, les malades devant se rendre à un dispensaire une fois par mois pour recevoir leurs medicaments oraux, même pendant les premiers 3 mois de traitement.
En 1979, un nouveau programme de service rural de chimiothérapie complétement supervisée a été introduit pour une période d'essai d'un an. Les malades devaient être admis à l'hôpital initialement pour une période de 2 mois avec un traitement quotidien, par streptomycine, isoniazide, rifampicine, pyrazinamide et ethambutol suivi par l'association une fois par semaine de streptomycine, isoniazide, rifampicine et ethambutol jusqu'au 12è mois, totalement supervisé dans les dispensaries de soins de santé primaires.
Les résultats thérapeutiques obtenus avec ce nouveau régime ont été excellents. Parmi les 125 malades dont le cas pouvait être évalué jusqu'au 30è mois (101 sujets présentant initialement un ou plusieurs frottis ou cultures positifs et 24 dont l'expectoration était négative au frottis et à la cuture), il n'y a pas eu d'échec bactériologique pendant la chimiothérapie et il y a eu une seule rechute après l'arrêt du traitement. Cependant, le traitement a présenté des problémes opérationnels majeurs en ce sens que 41 % des 263 malades traités au cours de cette année-là ont été traités par le régime standard, qui n'était pas totalement supervisé, au lieu du regime d'étude, 37 % à cause de leur refus d'hospitalisation initiale. Les résultats thérapeutiques ont été bien moins satisfaisant avec le régime standard.

El tratamiento estándar ha consistido en un esquema diario de 18 meses de duración, autoadministrado, que incluye isoniacida y PAS sódico suplementado con una inyección diaria de estreptomicina durante los 3 primeros meses. Los pacientes concurren al dispensario una vez al mes para solicitar los medicamentos orales, aún en los 3 primeros meses.
En 1979 se inició un nuevo programa rural de quimioterapia totalmente supervisado por un período de estudio de 1 año. Los pacientes eran hospitalizados inicialmente por un período que iba hasta dos meses, para un tratamiento diario con estreptomicina, isoniacida, rifampicina, pirazinamida y etambutol. El tratamiento continuaba hasta completar 12 meses, con estreptomicina, isoniacida, rifampicina y etambutol una vez por semana, completamente supervisado en el dispensario de atención de la salud.
Los resultados obtenidos con este nuevo esquema terapéutico fueron excelentes. De 101 pacientes con 1 ó más frotis o cultivos positivos y 24 con todos los frotis y cultivos negativos antes del tratamiento, los cuales fueron seguidos durante 30 meses, ninguno tuvo fracasos bacteriológicos durante el tratamiento y sólo 1 tuvo una recaída bacteriológica después de su suspensión. Sin embargo el esquema tuvo un importante inconveniente operacional ya que 41 % de los 263 pacientes tratados durante el ano tuvo que ser sometido a la quimioterapia estándar, la que no es completamente supervisada, en lugar del esquema estudiado; en 37 % de los casos se trataba de un rechazo del período inicial de hospitalización. Los resultados terapéuticos fueron mucho menos satisfactorios con el esquema estándar.
Ethambutol plus isoniazid for the treatment of pulmonary tuberculosis-A controlled trial of four regimens
1981, TubercleA controlled clinical trial of 4 regimens was undertaken in patients with bacteriologically positive, newly-diagnosed pulmonary tuberculosis. The regimens were: ethambutol 15 mg/kg plus isoniazid 400 mg, daily (E7H7) ; ethambutol 45 mg/kg plus isoniazid 15 mg/kg, twice a week (E2H2) ; ethambutol 90 mg/kg plus isoniazid 15 mg/kg, once a week plus isoniazid 15 mg/kg, mid-way between the weekly doses (E1 H2) ; and ethambutol 90 mg/kg plus isoniazid 15 mg/kg, once a week (E1H1). All patients received streptomycin 1 g plus ethambutol 25 mg/kg body-weight plus isoniazid 400 mg daily for the first 2 weeks. The total duration of treatment was 12 months for all patients.
There were 484 patients admitted to the study. After excluding 60 (41 with initial drug resistance to isoniazid), there remain 424 patients (107 E7H7, 101 E2H2, 107 E1H2, 109 E1H1) in the main analyses. The pretreatment characteristics of the 4 groups were broadly similar.
A favourable response at 12 months was observed in 96%, 88%, 93% and 75% of patients respectively: the differences between the E,H, regimen and the other 3 regimens were all significant (P≤0.03) as was that between the E7H7 and E2H2 regimens (P=0.05).
Among the slow inactivators of isoniazid, the proportions with a favourable response at one year were similar in the 4 groups (range 95-91 %). However, among the rapid inactivators, the proportion with a favourable response in the E1H1 group was only 57%. There was suggestive evidence that the E1H2 regimen was superior to the E2H2 regimen.
Of the patients with bacteriologically quiescent disease at 1 year, approximately a half, at random, had no further chemotherapy and were followed up for a 4-year period. Bacteriological relapse requiring retreatment occurred in 15 % of 54 E7H7 patients, 26 % of 38 E2H2, 33 % of 43 E1H2 and 54 % of 37 E1H1 patients, a significant difference (P<0.001). A final evaluation of long-term (5-year) favourable response achieved by the 12-month regimens was 83 % for the E7H7, 63 % for the E1H2 and 33 % for the E1H1 regimens.
In general, the regimens were well tolerated and the incidence of adverse reactions to the drugs was low. Of the 424 patients, 6 (1.4 %) developed a visual disturbance during the year of chemotherapy.
Un essai thérapeutique contrôlé, comportant 4 régimes, a été entrepris chez des malades atteints de tuberculose pulmonaire bactériologiquement confirmée et nouvellement diagnostiquée. Les régimes etaient: ethambutol 15 mg/kg plus isoniazide 400 mg chaque jour (E7H7) ;ethambutol 45 mg/kg plus isoniazide 15 mg/kg deux fois par semaine (E2H2) ; ethambutol 90 mg/kg plus isoniazide 15 mg/kg, une fois par semaine (E1H2) ; plus isoniazide 15 mg/kg une fois entre les deux doses hebdomaclaires and ethambutol 90 mg/kg plus isoniazide 15 mg/kg, une fois par semaine (E1H1). Tous les malades ont reçu de la streptomycine 1 g, de l'ethambutol 25 mg par kilo de poids coporel et de (l'isoniazide au cours des deux premieres semaines. La durée totale du traitement était de 12 mois pour tous les malades.
Au total, 484 malades ont été admis clans l'etude. Après exclusion de 60 malades (dont 41 porteurs d'une resistance initiale a l'isoniazide), il restait 424 malades (107 E7H7, 101 E2H2, 107 E1H2, 109 E1H1) dans I'analyse principale. Les caractéristiques avant traitement des malades des 4 séries etaient similaires.
Une reponse favorable à 12 mois a été observée chez 96 %, 88 %, 93 % et 75 % des malades, respectivement. La difference entre le regime E, H, et les 3 autres régimes était toujours significative (P⩽0,03), de même que celle entre les regimes E7H7 et E2H2 (P=0,05).
Ches les inactivateurs lents de l'isoniazide, les proportions de sujets présentant une response favorable à un an etaient similaires dans les 4 series (s'étalant de 95 à 91 %). Parmi les inactivateurs rapides, la proportion de sujets présentant une réponse favorable clans la série E, H, était de 57 % seulement. II semblait que le régime E, H2 était supérieur au régime E2H2.
Parmi les malades présentant e 1 an une tuberculose quiescente du point de vue bactériologique, environ la moitié, choisis au hasard, Wont pas reçu d'autre chimiothérapie et ils ont été suivis pendant 4 ans. Des rechutes bactériologiques nécessitant le traitement sont apparues chez 15 % des 54 malades E,H,, chez 26 % des 38 malades E2H2 chez 54% des 37 malades E, H,, difference significative (P<0,001). Une évaluation a long terme (5 ans) de la reponse favorable obtenue par le régime original de 12 mois a montré des taux de 83 % pour le regime E7H7, de 63 % pour le régime E2H2, de 63 % pour E, H2 et de 33 % pour E1 K.
D'une manière générale, les regimes ont été bien tolérés et l'incidence des réactions seconclaires aux medicaments a été faible. Parmi les 424 malades, 6, soit 1,4 %, ont vu apparaître une anomalie oculaire au cours de l'année de chimiothérapie.
Se realizó un estudio clínico controlado de 4 esquemas terapéuticos en enfermos de tuberculosis pulmonar bacteriológicamente positivos, recientemente diagnosticados. Los esquemas : etambutol 15 mg/kg mes isoniacida 400 mg, diariamente (E7H7) ; etambutol 45 mg/kg más isoniacida 15 mg/kg, dos veces por semana (E2H2) ; etambutol 90 mg/kg mes isoniacida 15 mg/kg, una vez por semana más isoniacida 15 mg/kg dada una vez entre dos dosis semanales (E, H2) ; y etambutol 90 mg/kg mes isoniacida 15 mg/kg, una vez por semana (E,H,). Todos los pacientes recibieron 1 g de estreptomicina mes etambutol 25 mg/kg de peso, más isoniacida 400 mg diarios, durante las dos primeras semanas. La duración total del tratamiento fue de 12 meses para el conjunto de los enfermos.
Fueron incluidos en el estudio un total de 484 pacientes. Después de excluir 60 (41 con resistencia inicial a la isoniacida) quedaron 424 pacientes (107 E,H,, 101 E2H2, 107 E1H2, 109 E,H,) para los análisis finales. Las caracteristicas de los enfermos antes del tratamiento eran, en grandes líneas, similares.
A los 12 meses, se observó una respuesta favorable en 96 %, 88 %, 93 % y 75 %, respectivamente; las diferencias entre el esquema E, H, y los otros 3 esquemas fueron todas significativas (P≤0,03), como igualmente entre los esquemas E7H7 y E2H2 (P= 0,05).
Entre los inactivadores lentos de la isoniacida, la proporción con respuesta favorable al termino de 1 ano, fue similar en las 4 series (variación 95-91 %). Entre los inactivadores rápidos, la proporción con respuesta favorable en la serie E, H, fue de sólo 57 %. Hubo evidencias que sugieren que el esquema E7H7 fue superior al esquema E2H2.
De los pacientes con enfermedad bacteriológicamente inactiva después de un año, aproximadamente la mitad, elegidos al azar, no prosiguieron otra quimioterapia y fueron sometidos a un seguimiento por un período de 4 anos. Las recaídas bacteriológicas que necesitaron un retratamiento ocurrieron en 15 % de 54 pacientes con el esquema E,H,, 26 % de 38 con E2H2, 33 % de 43 con E, H2 y 54 % de 37 con E1H1, siendo la diferencia significativa (P<0,001). La evaluación de la respuesta favorable a largo plazo (5 anos) lograda por el esquema terapéutico original de 12 meses dio como resultados : 83 % para el esquema E7H7, 63 % para E2H2, 63 % para E2H2 y 33 % para E1H1.
En general los tratamientos fueron bien tolerados y la incidencia de las reacciones desfavorables secundarias a los medicamentos fue baja. De los 424 pacientes, 6 (1,4 %) presentaron una afección ocular durante el ano en que se administró la quimioterapia.
