The term EA encompasses mild to moderate manifestations of lower respiratory tract inflammation and hyperreactivity and was previously known as inflammatory airway disease,28 and the severe phenotype of equine asthma (sEA) was previously referred to as heaves or recurrent airway obstruction.29–31 Hallmarks of sEA, which mainly affects horses older than 7 years, include marked neutrophilic lower airway inflammation, cholinergic bronchospasm, mucus accumulation with frequent coughing, increased respiratory effort at rest, and exercise intolerance.

The airways of horses affected with heaves shows many of the pathophysiological features of extrinsic human asthma, including chronic inflammation, airway obstruction induced by the inhalation of mouldy hay, and AHR to histamine and methacholine [157–160]. Inflammation in heaves predominantly involves neutrophils and in this regard may represent some aspects of chronic obstructive pulmonary disease (COPD) in humans.
