    It was designed to prove that sublobar resections would not be inferior to lobectomies with regard to local recurrence and cancer-free survival, but reported a 3-fold increase in local recurrence and a nonsignificant decrease in overall survival following sublobar resection. Despite the fact that a subsequent analysis decreased the significance of these differences,2 this study standardized lobectomy as the treatment of choice for stage IA NSCLC and has guided surgical care for the past 20 years. There is a slow recognition that the findings from the landmark trial may now belong to a different era.
