Overall, patients with missing data for BMI represented 14% of patients and for smoking status, 2% of patients. The BMI categories, determined from data closest to the index date, were defined as follows: underweight, <18.5 kg/m2; normal weight, ≥18.5 kg/m2 to <25 kg/m2); overweight, ≥25 kg/m2 to <30 kg/m2; and obese, ≥30 kg/m2 for patients ≥18 years old. (For children BMI was not calculated because accurate information on age in months required to calculate BMI z-scores was not provided for privacy reasons.)
