    The choice of needle length may have to be adjusted in patients with thick thoracic walls. A CT-based study of 53 Scottish patients found that the depth to the pleura was >4.5 cm in the mid-hemithorax line in the second intercostal space in up to 13.2% and up to 47.2% in the mid-axillary line in the fifth intercostal space.27 This result needs to be interpreted with caution because the tissue thickness in the mid-axillary line will be increased in the supine position owing to soft tissue falling to the side by gravity.
