A study of two twice-weekly and a once-weekly continuation regimen of tuberculosis chemotherapy, including a comparison of two durations of treatment 2. Second report: The results at 36 months

Abstract

The final results of a controlled clinical trial of 3 chemotherapeutic regimens in pulmonary tuberculosis in Czechoslovakia are presented. Two of the regimens were based on a twice-weekly continuation regimen of streptomycin plus isoniazid, preceded by 6 weeks or 13 weeks of initial daily triple chemotherapy with streptomycin, PAS and isoniazid; the third regimen was once-weekly streptomycin plus isoniazid preceded by 13 weeks of the daily triple combination. Patients in each regimen were allocated to a total duration of treatment of 12 months or 18 months.

At 3 years, the twice-weekly continuation regimen was highly satisfactory, 97 % of 93 patients receiving 13 weeks of initial daily triple chemotherapy and 98 % of 82 receiving 6 weeks having a favourable response. The once-weekly regimen was less satisfactory and this was largely due to the rapid acetylators of isoniazid of whom 78 % of 37 had a favourable response compared with 97 % of 60 slow acetylators. There was no advantage in the longer course of chemotherapy, for 95 %,of 129 patients receiving chemotherapy for 12 months had a favourable response compared with 95 % of 143 patients receiving it for 18 months.

Résumé

Présentation des résultats finaux d'un essai clinique réalisé en Tchécoslovaquie concernant trois régimes thérapeutiques pour le traitement de la tuberculose pulmonaire.

Deux des régimes avaient pour base un régime de continuation par streptomycine et isoniazide administrés deux fois par semaine; il était précéde par une chimiotherapie triple initiale quotidienne comportant streptomycine, PAS et isoniazide d'une durée de 6 semaines ou de 13 semaines; le troisième régime était un régime comportant la prise hebdomadaire de streptomycine et isoniazide, après 13 semaines de prise quotidienne de I'association triple. Les malades de chaque groupe thérapeutique recevaient le traitement pour une duree totale de 12 mois ou de 18 mois, par répartition au hasard.

A la fin de la troisième année, le régime de continuation hebdomadaire se montrait très satisfaisant : la réponse au traitement était favorable chez 97 % des 93 malades ayant 13 semaines de chimiothérapie triple initiale et chez 98 % des 82 sujets ayant reçu cette même chimiothérapie pendant 6 semains. Le régime hebdomadaire est apparu moins satisfaisant: ceci était surtout le fait de la présence de sujets acetylateurs rapides de l'isoniazide (37 sujets) dont 78 % ont presence une reponse favorable contre 97 % chez les 60 acetylateurs lents. II nest pas apparu que le traitement le plus long possédait des avantages; en effect, 95% des 129 malades ayant reçu 12 mois de chimiothérapie présentaient une réponse favorable centre 95% des 143 malades ayant reçu ce même traitement pendant 18 mois.

Resumen

Se presentan los resultados finales de una experiencia clínica controlada sobre 3 regímenes quimioterapéuticos aplicados a la tuberculosis pulmonar, en Checoeslovaquia. Dos de los regímenes se basaron en un esquema bisemanal de estreptomicina más isoniacida, precedido por 6 semanas o 13 semanas de quimioterapia triple diaria inicial con estreptomicina, PAS e isoniacida. El tercer régimen consistió en estreptomicina más isoniacida una vez por semana, precedido de 13 semanas de combinación triple, diaria. Se asignó a los pacientes sometidos a cada uno de los regímenes una duración total de tratamiento de 12 o de 18 meses.

A los 3 años, el régimen de continuación bisemanal fue abemente satisfactorio. El 97 % de 93 pacientes que recibieron 13 semanas de quimioterapia triple diaria inicial y el 98 % de 82 pacientes que recibieron sólo 6 semanas, tuvieron una respuesta favorable. El régimen de una vez por semana fue menos satisfactorio y 6sto se debió, en gran parte, a los acetiladores rápidos de isoniacida, entre quienes sólo el 78 % de 37 pacientes responsió favorablemente, comparado con el 97 % de 60 acetiladores lentos. La mayor duración del tratamiento quimioterapéutico no tuvo ventajas, ya que 95 % de 129 pacientes que recibieron quimioterapia durante 12 meses respondió favorablemente, en comparación a 95 % de 143 pacientes que la recibieron durante 18 meses.
